Home Medicines Review
This Program will promote opportunities
for inter-professional collaboration between
GPs and pharmacists for consumer medication
management in the community. The
Commonwealth Dept of Health & Ageing
through the Pharmacy Guild of Australia
provided funding for this program.
The goal of the HMR Program is to
maximise an individual consumer’s benefit
from their medication regimen, and prevent
medication-related problems through a team
approach, involving the patient’s GP and
preferred community pharmacy, with the
patient as the central focus. For details of
