The Division will work with and support GPs who address chronic conditions by providing appropriate knowledge on the mentioned risk factors, skills and resources (eg. electronic and hard copies of risk factor assessment and prescription)

 

Life Scripts

Lifescripts provides a framework for GPs, Practice Nurses and staff in the general practice setting to bring lifestyle risk factors to the fore in their engagement with patients.

Implementing Lifescripts involves: discussing risk factors with patients; setting lifestyle change goals; providing written lifestyle prescriptions; organising ongoing review of lifestyle risk factors; and referring patients to other services that support healthy lifestyle choices. Lifestyle Prescriptions is funded by the Australian Government Department of Health and Ageing, Lifescripts utilises the SNAP framework developed by the Royal Australian College of General Practitioners.

CALD Lifescripts – Culturally and linguistically diverse translated and adapted resources in the six most commonly spoken languages other than English in Australia.

Indigenous Lifescripts – Lifescripts resources that have been adapted for use with Aboriginal and Torres Strait Islander patients.

Pregnancy Lifescripts  - Pregnancy Lifescripts are now available The aim of the Pregnancy Lifescripts resources is to provide general practitioners with simple, brief evidence - based tools to assist in the provision of structured, consistent lifestyle advice on smoking, alcohol use and nutrition to pregnant women and women contemplating pregnancy.

Pregnancy Lifescripts Assessment and Prescription pad templates and Evidence Card are available to download from AGPN Website and save into Medical Director or you can order hard copy resources from General Practice South.

Lifescripts Case Studies 

To see examples of how Divisions of General Practice are implementing Lifescripts, Australian General Practice (AGPN) has developed a series of case studies that document a range of examples of implementation activities.

The case studies are available in the Lifescripts Resource Library. To open the case studies change the category to 'Case Studies'. 

Lifescripts Software 

Lifescripts software is currently being developed and will be available for distribution to General Practices. But you can download LifeScripts Medical Director Templates at the AGPN Website, (Select Lifescripts Templates). Alternately you can order Hard Copy Resources from General Practice South.
